Eye Test Reimbursement Form
The University of Otago General Staff Collective Employment Agreement and many General Staff Individual Agreements include provision for employees to claim reimbursement for the cost of an eye test, and for the purchase or replacement of lenses and frames where the need for spectacles is caused, or exacerbated, through the use of VDUs in the course of their normal duties.
Employees must have completed at least six months service at the time of the eye tests, and must be engaged in VDU duties for at least 50% of their normal working hours per week.
In any two-year period, not more than one claim will be considered as follows:
- Eye test (actual cost of standard examination, excluding retinal scans/photographs, optic nerves tests, etc.)
- Lenses (including contact lenses) up to $150.00, excluding photo-chromatic lenses, tinted lenses, etc.
- Frames up to $150.00 (if requiring spectacles for the first time or if lenses prescription requires new frames)
