Title: Workload Management of a primary health centre through prediction using care-plans for LTCs

An increase in numbers of people with long-term conditions (LTCs) pose a major challenge on primary health care (PHC) setting. In this talk, I address how this growing demand for care can be predicted, using care plans, at a population-level and be used for effective and efficient workload management of a PHC setting. The underlying data model will be briefly explained. I will also present the challenges and assumptions made to develop this rule-based workload simulation model. A few results and findings will be discussed as well.
