This form should be used to request the loan of a class set of iPads (based in Dunedin and Wellington), or an individual iPad Mini (Dunedin only), from ITS. Once you have completed this form, your request will be sent to ITS for approval, or you will be contacted if further clarification is needed.
Please note that:
- any bookings made with this form are only provisional and not confirmed until ITS has contacted you
- if you need to have the devices sent to your location, please allow an extra two days in your booking period to allow for freighting time
- devices can only be booked two days in advance to allow our staff time to prepare the devices for your loan
Please contact AskOtago if you have any questions about this form or do not receive confirmation of your loan within four working days.
Loan durations
In order that these devices can be made as available as possible to all borrowers, the following loan duration restrictions apply:
- two class sets - 14 days
- one class set - 28 days
- individual iPad Mini - 21 days
