Use this form to request the loan of a class set of iPads or an individual iPad Mini from ITS. Upon successful completion of this form the request will be sent to ITS for approval or further clarification where needed.
Please note any booking made with this form are only provisional and not confirmed until ITS has made contact with you. If you have had no response to your request after 3 working days please contact the ITS Service Desk.
Devices can only be booked 2 days from today's date. This is to allow our staff to prepare the devices for your loan. If you urgently require the iPad devices a loan may be possible. Please contact AskOtago for assistance.
If you need to have the devices transported to your location please allow an extra two days in your booking period to allow for freight lead times.
Device Allowances:
To make the devices as available as possible to all users the following loan length restrictions are in place:
- 2 Class Sets - 14 days
- 1 Class Set - 28 days
- iPad Mini - 21 days
