Registration can be submitted by printing the Registration form, completing and returning by fax or post to UNESCO Conference, Bioethics Centre, PO Box 913, Dunedin, New Zealand. Fax +64 3 474 7601. Payment accepted by cheque or credit card.

Registration will be confirmed by email unless otherwise requested. Please ensure that when paying by credit card an authorising signature is included beside 'cardholder signature'. This registration form becomes a tax invoice upon payment, please keep a copy for your own receipting purposes. A late fee charge of NZ$50 will apply to all registrations received after 19 January 2006. Cancellation of registration prior to 25 January 2006 will incur a $25 admin fee and after this date no refunds can be made for registration or accommodation.
