The Lordes supper, is Christes institution pure |
| In the which, breade and wyne geuen & his worde adioyned | |

| Set furth now, that all they which haue a faythfull mynde | |
| In these wordes, broke and shed for you, with a lowe kinde | |

The occasion whi Christ this supper dyd begyn |

The order of this supper who-soeuer do breake |
