Clinical Lecturer External Username Request Form

Only School of Medicine Clinical Lecturers who are not entered into the University payroll system should use this form to request a University username for access to Library eResources from off-campus (and ID card if required).

Clinical Lecturers are medical/health professionals whose primary employer is not the University, but who have a formal agreement with the Health Sciences Division, University of Otago Wellington, University of Otago Christchurch or Dunedin School of Medicine to provide teaching activities.

The term Clinicial Lecturer is used generically here to refer to the positions of Clinical Lecturer, Clinical Senior Lecturer, Clinical Associate Professor, Clinical Professor and/or Clinical Reader.
