The vast array of medications available to treat diseases makes it very important that pharmacists have the depth of understanding of pharmacotherapy in order to serve as knowledge navigators for patients and other health professionals. - This paper involves studying specific disease areas and the use of evidence-based pharmacotherapy in the treatment of diseases and preparation of patient-centric care plans for actual patients, which includes information gathering, patient assessment, evaluation of all aspects of pharmacotherapy, care planning, communication of the plan, and follow up.
