Student Absences and Impairments

To request approval for planned leave of absence from classes and clinical practice

Your application for planned leave (for example, family events, sports leave, cultural events) must be submitted prior to making travel arrangements. We will not process applications if written verification for the event is not attached and provided in advance.

  1. Print the Application for Leave form (PDF, 225 KB)
  2. Complete and sign the form
  3. Attach verification (for example family member verifying wedding or graduation of a sibling, letter from coach or sports administrator)
  4. Scan (or photograph) the completed and signed form and verification
  5. Write us an email
  6. In the subject line write Leave
  7. Make sure that all labs and clinical placements you would be missing are clearly identified
  8. Email all files and attachments to absence.physio@otago.ac.nz

If you are ill, or unexpectedly have to miss classes

You must advise the School of Physiotherapy before 8am on the day of your absence.

Follow this process;

  1. Write us an email
  2. In the subject line write Absent
  3. Add your year of study and your student ID number
  4. Add reason for absence
  5. Tell us what specific clinical placement or lab you should be attending that day
  6. Tell us if you will be away all day or only part of the day
  7. Email us at absence.physio@otago.ac.nz

Important notes

  • If you are ill and away for more than three days, you must supply a medical certificate, along with the Application for Leave form.
  • If you are on clinical placement, phone or email your clinical educator at least 30 minutes before the start of the placement to inform them of your absence.
  • When you recover, please download (if you are ill and away for three days or less) and email the completed Application for Leave form (as above) on the first day of your return to classes and clinical practice. No further documentation is required.

Unplanned absence due to unexpected event such as bereavement-tangihanga

You must advise the School of Physiotherapy before 8am on the day of your absence.

Follow this process;

  1. Write us an email
  2. In the subject line write Absent
  3. Add your year of study and your student ID number
  4. Add reason for absence
  5. Tell us what specific clinical placement you should be attending that day
  6. Tell us if you will be away all day or only part of the day
  7. Email all files and attachments to absence.physio@otago.ac.nz

Important notes

  • If you are on clinical placement, phone or email your clinical educator at least 30 minutes before the start of the placement to inform them of your absence.
  • When you return to classes, submit the Application for leave form and written verification of the reason, such as an email from your parents or relevant source.

If you are ill, or unexpectedly have to miss a test or other form of assessment

Firstly, you must advise the module or paper coordinator by email prior to the due date or within two days of the due date of the assessment.

Follow this process;

  1. Print the Application for Impairment for Internal Assessment form (PDF, 110 KB)
  2. Write us an email
  3. In the subject line write Impairment
  4. Add your year of study and your student ID number
  5. Scan (or photograph) and attach completed and signed form
  6. Scan (or photograph) and attach a medical certificate or other verification
  7. Email all files and attachments to absence.physio@otago.ac.nz

If you are ill or impaired during a mid year assessment or final examination

Firstly, you must advise the School of Physiotherapy before 8am on the day of your absence.

Follow this process;

  1. Write us an email
  2. In the subject line write Special
  3. Add your year of study and your student ID number
  4. Tell us what exam you will be missing, and the reason
  5. Email us at absence.physio@otago.ac.nz

Important notes

  • Then, within 48 hours of your mid-year assessment or final exam you must submit the Mid-year assessment final exam Special Consideration form, along with medical certificates and verification.

Follow this process;

  1. Print the Mid-year assessment final exam Special Consideration form (PDF, 169 KB)
  2. Scan (or photograph) the completed and signed form
  3. Scan (or photograph) and attach a medical certificate
  4. Write us an email
  5. In the subject line write Impairment
  6. Add your year of study and your student ID number
  7. Attach both scanned (or photographed) files
  8. Email all files and attachments to absence.physio@otago.ac.nz

Important notes

  • You must submit both forms to the School of Physiotherapy within 48 hours of your last examination/assessment.
  • The Special Consideration application will be considered by the Board of Examinations and Assessments.
  • You will be notified whether or not your application for Special Consideration was successful. If the application is accepted, you will be offered the opportunity to sit a special examination at a later date.
