PHCY522 Applied Clinical Pharmacy 1

Medication in the treatment of diseases including cardiovascular and respiratory disorders, diabetes, and rheumatological, gastrointestinal, renal, and hepatic disorders.

At a time when other health professionals - and patients - have access to Dr Google, it is very important that you have the depth of understanding of pharmacotherapy that you can be knowledge navigators for other health professionals and patients. Your unique input into improving health outcomes is your depth of knowledge and understanding, which enables you to optimise medicines therapy and so reduce drug-related morbidity and mortality. This paper helps pharmacists to become active participants in the health care team, contributing to optimising medicines-related health outcomes and being recognised as having specialised knowledge.

Paper Structure
This paper involves the preparation of patient-centric care plans and pharmacist plans of care, including information gathering, patient assessment, evaluation of all aspects of pharmacotherapy, care planning, communication and follow up.
Teaching Arrangements
One compulsory 2-day weekend workshop in February and five 2-hour evening videoconferences.

Learning Outcomes
Students who successfully complete the paper will
  • Gain a solid understanding of the pathophysiologic basis of common diseases and disorders
  • Discuss, in depth, the pharmacological basis of current therapeutics
  • Highlight and evaluate areas of therapeutic controversy and new drug developments
  • Become fluent and confident in the use of clinical terminology
  • Be able to quantify likely adverse effects and interactions in terms of the relevance to an individual
  • Apply pharmacotherapy knowledge and problem-solving skills to improve the medication management of an individual's medicines, accounting for all factors influencing the person's medicines use
  • Communicate clear and concise therapeutic information in both oral and written form

Medication in the treatment of diseases including cardiovascular and respiratory disorders, diabetes, and rheumatological, gastrointestinal, renal, and hepatic disorders.

Other health professionals - and patients - have access to extensive information on health issues, including medicines, via the Internet and social media. This means that it is very important that you have a depth of understanding of pharmacotherapy so that you can be knowledgeable navigators for other health professionals and patients and deal with the complexity of multiple co-morbidities and multiple medicines. Your unique input into improving health outcomes is your depth of knowledge and understanding, which enables you to optimise medicines therapy and so reduce drug-related morbidity and mortality, particularly when managing complex patients when there is no clear guideline-driven therapy. This paper helps pharmacists to become active participants in the health care team, contributing to optimising medicines-related health outcomes through clear assessment, clinical reasoning and the ability to justify clinical decisions, and being recognised as having specialised knowledge to make a unique contribution.
