Replacement degree or diploma certificate


Replacing your qualification certificate

Your hard-copy University of Otago degree, diploma, or certificate is a legal document, therefore there can be only one in existence at any time.

If you are still in possession of your original certificate - even if it is inaccessible to you - we are unable to provide you with a replacement copy of your certificate.

If your certificate was issued after September 2017, a verified digital version is available via the My eQuals portal.

If your certificate was issued prior to September 2017, please contact the Graduation Office to enquire about obtaining a digital copy via the My eQuals portal.

If your hard-copy certificate has been lost, damaged, or destroyed this can be replaced.  To obtain a replacement you need to complete the form and return it along with your payment, and your Statutory Declaration, to the University at the address below.  Please do not double-side your forms.

Cost and timeframe

Replacement certificates cost $75 (plus courier) per qualification, and take 4 to 6 weeks to process.

Statutory declaration

Included with the replacement form is a Statutory Declaration.  This needs to be witnessed and stamped by one of the following in order to verify your reason for requesting a replacement.

  • Justice of the Peace
  • Solicitor
  • Notary Public
  • Commonwealth Representative (for those outside of NZ)
  • or Officer authorised to take and receive Statutory Declarations

Damaged certificates

If your original certificate still exists but has been damaged, you must return the damaged certificate to the University along with your forms.

Returning your completed forms

As we need to receive the original Statutory Declaration, we are unable to receive forms by fax or email.  Please do not double-side your forms.
