Easy access to frequently used forms at Student Health
- Student Health Enrolment Form (PDF) – This form can be completed and brought with you to your first appointment
- Student Health Medical History Form (PDF) – This form can be completed and brought with you to your first appointment
- Authority to Release Health Records (PDF) – Health records to be sent from Student Health to your GP
- Special Consideration for Final Exams – Information about special consideration for Final Exams
- Community Service Card (CSC) – Reduces cost of appointments e.g. Doctors from $25 to $10. If you get a Student Allowance, you will automatically be sent a card once your allowance is approved. But if you have a partner who's working, you'll need to apply.
- St John Supporter Scheme – a once a year one-off cost to access free Ambulance Services for the rest of that year
